RemedyElderCare provides patients with online access to a Registered Nurse Practitioner experienced in providing advice for patients with more than 1 disease (multiple co-morbidities). The program provides patients with live video consultation(s) with a Registered Nurse Practitioner who work with you to manage your illnesses. With the assistance of your Consultant you will jointly devise a customized care plan. Our consultants can provide a detailed assessment of your health, clear and concise education focused on empowering you to take control of your health.

Elder Care
Many seniors prefer to remain in their home rather than move to a care facility, this gives them a sense of independence and better quality of life. Frequent travel to visit health care professionals can be physically and emotionally taxing and Remedy Elder Care allows them to get exceptional care at home. RemedyElderCare also gives their families the confidence that they are receiving the highest level of care in their home.

RemedyElderCare provides support for these seniors through the Registered Nurse Practitioner(s) first help by assessing the following:

  • The capabilities of each patient (physical, mental and social skills);
  • The areas they need assistance in;
  • Their daily living skills
  • Compiling a home safety checklist
  • Medication list
  • Nearest resources for services (homemaking, pharmacy, social activities, computer support etc.)

Preparing a care plan together with the senior that meet their needs and, if required, the needs of their family

Develop a virtual follow-up visit plan to monitor:

  • Vital signs
  • Management of any chronic illness
  • Medication adherence
  • Identify/provide a list of local resources for specific tasks and activities
  • Other services as needed by the individual senior and their family
